HOW TO JOIN MOATFIELD SURGERY

Due to current pressures on the practice, we have taken the decision to limit the number of new patient registrations at Moatfield Surgery.

This means the patient registration process will operate in a different way and could involve registering with an alternative practice. 

For assistance with finding a GP Surgery in Sussex, please contact the NHS Sussex GP Allocations Teams by email sxicb.gpallocations@nhs.net stating the full names of everyone who needs to register, and your full address, including postcode. Alternatively you can call 0300 140 9855. The team will provide you with the available practie options and arrange for an allocation if necessary. 

However we will continue to register the following patients in the usual way:

  • Newborn or adopted babies of parents already registered at the practice who are still living within our boundary

  • First degree relatives of patients already registered at the Practice who are living at the same address as registered patients and are still living within our boundary

Moatfield Surgery operates within its published boundary and does not accept patients who reside outside this defined area. Patients should note that if your address is outside of this area you will be asked to register with another surgery. Please see our boundary area below.

Do you live within our Boundary Area?

Accountable / Named GP for All Patients

Since 2004, patients are registered with the practice not a specific GP. All patients registered at the surgery have a named doctor who has overall responsibility for your care and support. If you choose to see another doctor at the surgery you are entirely free to go on doing so exactly as before.

 

Disabled Patient Facilities

Disabled patient facilities are also available at this practice.
(Limited parking (wheel chairs, ramp and disabled toilets).
